The New York Department of Health performs inspections and responds to complaints for nursing homes across the state. While it has not pursued any enforcement actions against Windsor Park Rehabilitation and Nursing Center in the past 10 years, it has both responded to complaints and issued citations for various violations.

Winsor Park Rehabilitation and Nursing Center also had numerous citations for infection prevention and control, a safe/clean/comfortable environment, unnecessary psychotropic medications, and abuse and neglect.
