Troy Center for Nursing and Rehabilitation received 52 citations for violations of public health code between 2018 and 2022, according to New York State Department of Health records accessed on June 17, 2022. The Troy nursing home’s citations resulted from a total of six inspections by state surveyors. The violations they describe include the following:
1. The nursing home did not keep medication rates low enough. Section 483.45 of the Federal Code stipulates that nursing homes just ensure “Medication error rates are not 5 percent or greater.” A February 2021 citation found that Troy Center for Nursing and Rehabilitation failed to ensure such. The citation specifically describes an instance in which a medication pass resulted in an error rate of 30.77%. Among other errors, the citation states, a medication that should have been administered with food was not administered with food; in another instance, a resident’s medications were administered late. A plan of correction undertaken by the facility included the education of relevant staff.
2. The nursing home did not adequately protect residents from accident hazards. Under Section 483.25 of the Federal Code, nursing homes are required to provide residents with adequate supervision and assistance devices to prevent accidents. A March 2021 citation found that Troy Center for Nursing and Rehabilitation failed to ensure such. The citation states specifically that the facility failed to adequately supervise a resident “with a known history of wandering.” In an interview, a Licensed Practical Nurse said that the resident “was allowed to wander out of his room, and that there were no care planned interventions for supervising or monitoring” the resident. A plan of correction undertaken by the facility included the review and updating of the resident’s care plan.
3. The nursing home did not adequately prevent medication errors. Section 483.45 of the Federal Code stipulates that nursing homes must ensure that residents “are free of any significant medication errors.” A March 2021 citation found that Troy Center for Nursing and Rehabilitation failed to ensure such. The citation states specifically that a resident did not receive blood pressure medication as ordered. The citation also describes another resident woh did not receive anti-anxiety medication as ordered. A plan of correction undertaken by the facility included the education of the nurses responsible for the errors.
